A healthcare provider is seeking an Innovations Care Manager in North Carolina. This remote position requires travel within the state to coordinate care for members, develop patient care plans, and conduct health assessments. The ideal candidate will have a bachelor's degree in health-related fields and at least two years of experience in care management, particularly with individuals with intellectual/developmental disabilities or traumatic brain injury.
