Position Title: Physical Therapist (PT)
Location: Upland Hills Health - Home Health Services
Role & Department: Staff PT in the Upland Hills Health Home Health Services Department
Role Responsibilities:
- The Home Health Physical Therapist will do both evaluations and treatments of home care patients in their homes and provide services necessary for the patients
- Will design, organize and conduct and/or supervise the performance of subordinate personnel of medically prescribed therapy treatment programs and education
- The Physical Therapist evaluates, records and reports patient problems and progress, providing effective communication with other members of the care team
- Will assist in developing new services/programs as needed and assess the need for such services
Candidates must be licensed in the State of Wisconsin as a Physical Therapist
Prior work experience as a treating therapist is preferred
Prior home health care experiences are beneficial
Experience with EMR's is preferred
Interest in professional growth as demonstrated by attendance at continuing education programs and participation in professional organizations
Ability to direct, implement and report on effectiveness of PT treatments
Previous experience with the Microsoft Office Suite software programs is beneficial
