• Provide direct supervision and clinical oversight for multi‑disciplinary teams supporting members with behavioral health conditions
• Ensure staff complete comprehensive needs assessments and care plans for members as care needs change
• Establish a team‑based, person‑centered approach to coordination of care
• Monitor teams to ensure integration of care through the establishment of a multi‑disciplinary team
• Oversee daily work schedules to confirm that individualized, person‑centered plans of care are developed and acted upon in a timely manner

• Master’s degree and three (3) years of experience in care management, case management, or care coordination for the MH/SU/BH population; OR
• Licensure as a Registered Nurse (RN) and three (3) years of experience in care management, case management, or care coordination for the MH/SU/BH population
• Fully licensed in the State of North Carolina as LCSW, LCMHC, LPA, LMFT, LCAS, or RN
• Must have a valid driver’s license
• Must reside within 75 miles of Trillium’s Catchment in North Carolina

• Experience in a managed care environment
• Familiarity with behavioral health services and community resources
• Strong leadership and team-building skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a remote setting
