What We Offer
The Medical Social Worker-Patient Advocate is responsible for assessing and implementing social work services in the home setting. Assists department team with difficult and complex social situations and provides social work interventions as appropriate. Benefits effective as of hire date. Relocation assistance provided. This is a hybrid position 50% at-Home, 50% in-home patient visits. Must have a Master's Degree in Social Work.
Work Location: Hybrid - Work from home and in-home patient visits (50/50%). Company car, gas car and laptop provided.
Work Status: Salaried/Exempt position.
Schedule: Tuesday - Friday, 8 am - 5 pm (32 hours week)

What You'll Do- The Medical Social Worker-Patient Advocate will provide education and contact information to patients and families regarding needed community resources.
- Provides psychosocial assessments and interventions to improve or maintain the social, emotional, and physical health status of the patient
Serves as a liaison between patients and external agencies. - Assess/provide assistance with community support programs
- Provide support with food or housing insecurity
- Provide transportation support program assistance
- Medicare application assistance
- Partner with RN/Case Manager and therapy team when necessary
- Education:
Graduate Degree, required. Graduate of a school of social work that is approved by the Council on Social Work Education with a Master’s Degree (MSW). - Experience:
Minimum of two years professional social work experience in a healthcare, hospital, or community health setting providing clinical services, required. Home health experience, preferred. - Licensure/Certification:
Valid NC Driver’s License, required. - Additional skills required:
Ability to successfully complete generic and department specific skills validation and competency testing; knowledge of chronic disease issues; knowledge of community resources; knowledge of department processes and needs. Demonstrate above average ability in the following areas: managing a complex and varied work load, understanding physical and psychosocial assessment parameters. Strong written and verbal skills; strong problem solving, decision making, organizational and critical thinking skills;
and ability to network and access appropriate resources.
