Our services (upon licensure) include Skilled Nursing for medication management and chronic disease monitoring, Physical Therapy for mobility and strength, Occupational Therapy for daily living safety, Speech Therapy addressing communication and swallowing, and Home Health Aide support under RN supervision.

This is a part-time hybrid Home Health Nurse role based in Longwood, FL, with some work from home flexibility. The Home Health Nurse will provide high-quality skilled nursing care to patients, including medication management, wound care, chronic disease monitoring, and personalized patient-centered services. Responsibilities include assessing patient needs, developing and implementing care plans, educating patients and caregivers, collaborating with therapy teams, and ensuring optimal outcomes through attentive and compassionate care.
Qualifications- Proven expertise in Nursing and Medicine, supported by an active nursing license
- Experienced in Home Care and providing personalized Patient Care
- Proficient in managing Wound Care and chronic disease monitoring
- Strong ability to communicate effectively with patients, families, and multidisciplinary teams
- Exceptional organizational and time management skills to manage a flexible, hybrid schedule
- Current CPR certification and a valid driver's license with reliable transportation
- Experience in home health or a similar healthcare setting is a plus
