Responsibilities
- Proactively contact eligible members to promote home based lab and medication refill services.
- Educate members on available services, eligibility criteria, and access channels.
- Encourage members to utilize approved service medical pathways to support continuity of care.
- Guide members on required steps, documentation, and timelines.
- Facilitate communication between members and healthcare service teams when needed.
- Assist in directing member inquiries to appropriate service channels.
- Support service awareness campaigns and outreach initiatives.
- Accurately document member interactions in approved systems.
- Maintain data confidentiality and comply with organizational data protection policies.
- Ensure all interactions are properly recorded for operational and regulatory compliance.
- Adhere to MOH, CCHI, and Bupa Care Connect policies and operational procedures.
- Maintain confidentiality of member information at all times.
- Escalate member concerns, complaints, or service issues according to defined escalation protocols.
