Behavioral Health Advocate - Rhode Island
Optum is seeking a full-time behavioral health care advocate to help individuals live their lives to the fullest by promoting recovery and resiliency via coordination and collaboration with multiple internal and external partners including consumers and their families/caregivers, medical, behavioral health and clinical network teams.
Hybrid Position: 25% of the week is spent in the community, meeting with members in hospitals, facilities, or homes. The remainder of the week is spent working remotely at home working telephonically. Typical hours are Monday - Friday, 8:00am - 5:00pm. Licensure and residency in the State of Rhode Island is required but candidate may reside anywhere in the state of Rhode Island.
Responsibilities- Engage and motivate consumers via a recovery, health and wellness-oriented approach
- Identify and remove barriers to services
- Provide follow-up, monitoring and education, in-person and telephonically
- Support a seamless transition for consumers from an inpatient setting to community-based services
- Attend monthly meetings with Community Mental Health Organizations' Healthy Homes and CCBMH's
- Active, unrestricted, independent behavioral health licenses to practice in the State of Rhode Island: LICSW, LMFT, LCMHC
- 2+ years of case management experience
- Dedicated workspace and access to high-speed internet via cable/DSL/fiber service in your home
- Driver's license and the ability to travel as needed within the designated service delivery area (up to 25%)
