Job Description

The Clinical Nurse Liaison plays a pivotal role in generating referrals to HSL Home and Community-Based Services, focusing primarily on Home Health and Hospice. This position is crucial for guiding patients into home-based services, thereby enhancing the patient and family experience.

The successful candidate will initiate and cultivate relationships within hospitals, post-acute facilities, physician practices, and assisted living communities. By coordinating care transitions effectively, the Clinical Nurse Liaison will ensure a seamless entry into HSL Home Care and Hospice services.

Key Responsibilities Include
  • Facilitating the transfer of patients from facilities to HCBS service lines, including home health and hospice, through collaboration with healthcare teams.
  • Providing liaison services to the Rehabilitation Services Unit, primarily via email and telephone, with occasional virtual or in-person meetings.
  • Tracking patient census and communicating potential referrals to HSL Home Care based on patient needs and insurance.
  • Updating home care staff with pertinent patient information and following up on hospitalizations.
  • Offering clinical insights for intake coordination and addressing patient concerns.
  • Maintaining effective communication with patients, families, and healthcare professionals.
  • Promoting patient education and community outreach regarding home care services.
  • Participating in the intake process to ensure thorough documentation of patient transfers.
  • Addressing safety concerns and contributing to compliance with regulations and standards.
  • Engaging in professional growth through educational opportunities.
Qualifications
  • Minimum of two years in community-based healthcare; experience in home health and hospice preferred.
  • Clinical licensure in Massachusetts strongly preferred.
  • Proven track record in healthcare sales or liaison work.
  • Exceptional verbal and written communication skills and relationship-building abilities.
  • Adaptability and a strong motivation to learn are essential.
  • Basic computer literacy is required.
