Location and Travel

Hybrid position: 3‑4 times per week onsite. Travel to patient homes within 25 miles of the practices in South Region—Boston, South Weymouth & Foxborough Patriot Place. Some practices are in Boston.

Job Summary

Mass General Brigham’s Population Health team is committed to enhancing the health care experience for older adults through personalized, home‑based care that supports independence, functionality, and quality of life. We develop innovative programs that deliver care where and when it is needed most. This patient‑centered model ensures care is tailored to the unique needs of each individual. By leveraging interdisciplinary collaboration and proactive care management, our value‑based care models address the complex needs of older adults.

This is an exciting opportunity to join the Mass General Brigham Population Health department as a Nurse Practitioner within the GRACE (Geriatric Resources for Assessment and Care of Elders) program. The GRACE Nurse Practitioner will provide home‑based geriatric assessment and care management for older adults aligned with Mass General Brigham’s Primary Care Providers. This role is designed to proactively support vulnerable older adults, ensuring comprehensive, coordinated care that aligns with their goals and preferences.

Key Responsibilities
  • Conduct in‑home geriatric assessments in collaboration with a GRACE Social Worker.
  • Develop and implement individualized care plans using GRACE care protocols.
  • Engage patients and caregivers in health education, chronic disease management, and goals‑of‑care discussions.
  • Coordinate care with primary care providers and the GRACE interdisciplinary team to optimize outcomes and reduce unnecessary hospital utilization.
  • Key Components of GRACE Team Care
    • In‑home assessments by a nurse practitioner and social worker team.
    • Individualized care plans developed using GRACE protocols.
    • Collaborative care planning with primary care physicians.
    • Ongoing home‑based care management and coordination.
    • Weekly interdisciplinary team meetings including a geriatrician, pharmacist, and mental health liaison.
    Qualifications
    • Master’s Degree in Nursing required.
    • Cannot accept experience in lieu of a degree.
    • Registered Nurse (RN — MA State License).
    • Nurse Practitioner (NP) / Advanced Practice Registered Nurse (APRN) — MA State License.
    • Experience as a nurse practitioner, 3‑5 years highly preferred.
    • Valid driver’s license and vehicle; commuting to patient homes is required.
    Skills for Success
    • Skilled in taking medical histories to assess medical condition and interpret findings.
    • Ability to maintain quality control standards.
    • Ability to react calmly and effectively in emergency situations; interpret, adapt and apply guidelines and procedures.
    • Ability to communicate clearly and establish/maintain effective working relationships with patients, medical staff and the public.
    Working Model Requirements
    • M-F Eastern Business hours.
    • South Region: must be accessible 4‑5 times weekly to South Weymouth Main St., and Foxborough Patriot Place practices.
    • Must travel to patient homes within 45 miles of MGB practices. Frequent travel to practice locations and remote work approved as needed.
    • Remote: 1 day per week likely, based on business needs; must have a quiet, compliant workspace for remote work.
