Job Title: Remote Program Coordinator/ Health Service Program Coordinator - Diabetes and Cardiovascular Health

Location: Providence RI 02908

Pay Rate: $31.80 /hour on w2 without benefits

Duration: 2+ Years

Shifts Timing: 15-20 hours week / approx 3 hours a day Monday-Friday. Flexible hours in the AM and PM between 7:30am - 5:30pm. Mainly remote with occasional in-person meeting and or trainings, as needed.

Job Description
  • Client has retained Knowledge services to provide a Lifestyle Change Program Navigator to support the Center for Health Promotions Diabetes and Cardiovascular Health Program (DCHP).
  • The Lifestyle Change Program Navigator position is an integral part of DCHP's goal to increase access to, and utilization of, evidence-based chronic disease management and prevention programs supported by two Centers for Disease Control and Prevention (CDC) cooperative agreements.
  • The Lifestyle Change Program Navigator will use a data management and referral system to manage incoming referrals and engage individuals telephonically to facilitate enrollments into the National Diabetes Prevention Program re-brand in RI as Ready for Health and the Healthy Heart Ambassador Blood Pressure Self-Monitoring program.
Roles and Responsibilities
  • Conduct Health Related Social Needs (HRSN) screenings to identify and address program participation barriers.
  • Partner with the Rhode Island Diabetes and Cardiovascular Health Program (DCHP) to meet CDC deliverables associated with the Healthy Heart Ambassador Blood Pressure Self-Monitoring (HHA BPSM) Program and Ready for Health program enrollments.
  • Oversee the day-to-day referral and registration operations for the HHA BPSM program and Ready for Health in partnership with community-based and healthcare organizations across the state.
  • Assist with increasing access to, and availability of, the HHA BPSM program and Ready for Health program statewide for priority populations.
  • Track and monitor the HHA BPSM program and Ready for Health classes and cohorts across the state.
  • Communicate with referring healthcare systems and community-based organizations to follow-up with referrals and program participation.
  • Participate in telephonic and digital outreach to promote the HHA BPSM program and Ready for Health program across RI.
  • Assist with a digital marketing campaign for healthcare systems, community-based organizations, and employers promoting evidence-based programs and referrals.
  • Provide technical assistance and support to program delivery sites to improve use of the data management platform, as needed.
  • Participate in data collection and evaluation activities in partnership with DCHP evaluator.
  • Participate in stakeholder meetings, as needed.
  • Assist in the planning, organization, coordination, and administration of a health program.
  • Assist in monitoring the goals and objectives of the program.
  • Collect and monitor referral data using a HIPAA compliant platform and prepare reports related to the program's functions and activities.
  • Administer the provisions of laws, rules and regulations, as well as adhere to provisions set forth in grant and contractual agreements relating to the operation of a health program.
Required Skills
  • Bilingual Spanish required.
  • Strong customer service experience.
  • Operations management skills.
  • Strong data management skills.
  • Proficient with Microsoft TEAMS and Excel, including formulas and formatting.
  • Strong experience working with community-based organizations and/or in healthcare setting.
  • Certified Community Health Worker preferred, not required.
  • Commitment to the Department's Health Equity mission.
  • Knowledge of HIPAA compliancy and confidentiality.
Education and Experience
  • Bachelor's Degree with specialization in public health, public administration, business administration, hospital administration, or a related field.
  • Experience gained through employment in a responsible position involving a call center, customer service, hospital, public, business, or health administration.
  • Experience in providing technical assistance and support. OR any combination of education and experience that shall be substantially equivalent to the above education and experience.
