Overview
SENTARA REGIONAL RESOURCE POOL IS HIRING AN INPATIENT CASE MANAGER, RN – DAYS/PRN.

Role: Inpatient Case Manager, RN – Days/PRN
Responsibilities- Provide medical social work services to patients in the hospital setting, assessing social, psychological, cultural, environmental, and financial factors as well as disposition needs.
- Collaborate with patients, families, healthcare team members, and community organizations; advocate to identify needs and available resources in the hospital and community.
- Develop and evaluate effective transition plans considering diagnosis, prognosis, treatment course, past and present services, goals, provider options, and available health care benefits across the continuum of care.
- Functions in the hospital setting and may work across units within the case management specialty; required to work shifts in multiple hospitals within the Southside or Western Tidewater regions.
- Experience:
18 months of recent acute care RN experience. - Flexibility to work in 4 hospitals within designated regions; ability to cover various units as needed.
- Operational hours: 8:00 AM – 4:30 PM, 7 days per week.
- Flexi/PRN: prescheduled hours must total at least 48 hours per calendar month, with 16 hours including weekend availability.
- Education and orientation hours do not count toward prescheduled or work hours requirements.
Hospital region options (must be assigned to one region and available to work at any of the four assigned hospitals based on greatest need):
- Southside Locations: SNGH, SLH, SPAH, SVBGH — Norfolk and Virginia Beach
- Peninsula Locations: SOH, SCH, SWRMC, SAMC — Suffolk, Hampton, Williamsburg, Elizabeth City
- Western Tidewater: SNGH, SLH, SPAH, SVBGH; SOH, SCH, SWRMC, SAMC
As an Inpatient Care Manager, responsible for providing social work services to hospital patients; collaborates with care teams to coordinate discharge planning and transition of care.
This position offers high flexibility for hours and shifts and may include regional or local travel and enhanced pay incentives.
