Gloucester Inner City Primary Care Network is seeking adedicated and experienced Specialist Diabetes Lead Nurse to support the delivery of the PCNs Diabetes Enhanced Service. This is a unique opportunity to be part of an innovative, integrated approach to diabetes care, working closely with a multidisciplinary team including Diabetes Specialist GP,Practice Nurses, Pharmacists, Health Care Assistants, and secondary carepartners.
The role will primarily focus on providing clinical delivery, leadership and management of red-risk type 2 diabetic patients,improving outcomes in line with national treatment targets, and reducing risk stratification levels. The postholder will be instrumental in delivering proactive, patient-centred diabetes care with a strong emphasis on early intervention, medication optimisation and initiation, lifestyle support, and continuity of care. The role requires support and mentoring of Diabetes practice nurses across the PCN.
Working Hours
: 22.5 hours per week (3 days) 1 day work from home,2 days on site rotation across surgery sites across the month proportionate based on patient list size.

Main duties of the jobTo provide leadership to the PCN diabetes team including ancillary roles (HCAs, etc).
To foster an effective, quality driven PCN diabetes service.
To support the development of Gloucester Inner City PCNs diabetes services for registered patients of all practices within the PCN.
Work in compliance with CQC, statutory and NHS requirements applicable to the PCN diabetes service
To provide a high standard of specialist diabetes nursing care for patients, as well as providing diabetes nursing assistance to the doctors andother members of the primary healthcare team.
The duties will include all tasks normally undertaken by an experienced
RGN and any additional roles agreed between the nurse and practice management team as appropriate, having regard to the nurses training and competencies.

Job responsibilitiesDuties Specific to Role
Leadership
PCN diabetes nurse team leadership and supervision.
Lead, support and manage other members of the PCN diabetes team,including trainees and HCAs.
Delivera dedicated diabetes specialist Lead Nurse clinic targeting patients stratified as Red Risk under the PCNs diabetes risk model.
Review complex cases and provide specialist input into the diagnosis, monitoring,and treatment of patients with Type 2 diabetes.
Initiate and optimise pharmacological treatment including insulin, GLP-1 receptor agonists, SGLT2 inhibitors, and other oral hypoglycaemic agents, in accordance with NICE and local guidelines.
Manage follow-up pathways including blood testing, medication titration, and patient education.
Liaise with Diabetes Practices Nurses to determine clinic lists and monitor progress across the cohort.
