Overview
Sentara Health is looking to hire a Remote Transition Care Coordinator. This is a remote position open to candidates residing in the following states:
Virginia, North Carolina, Alabama, Delaware, Florida, Georgia, Idaho, Indiana, Kansas, Louisiana, Maine, Maryland, Minnesota, Nebraska, Nevada, New Hampshire, North Dakota, Ohio, Oklahoma, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Washington (State), West Virginia, Wisconsin, Wyoming.
The Regional Transition Care Coordinator (TCC) manages and supports all transitional care and post-acute services for members in a specific geographic region. Responsibilities include transitioning individuals from nursing facilities, hospitals, inpatient rehabilitation, or other institutional settings into the community, and assisting those who wish to remain within their community setting. The TCC collaborates with the Care Coordinator, interdisciplinary care team meetings, and facilities to facilitate a safe and effective transition.
The TCC plans, coordinates, monitors, and evaluates options to meet individual needs prior to transition and ensures a smooth handoff to the receiving Care Coordinator.

- Bachelor's degree in health & human services or related field (Preferred).
- Licensed Mental Health Professional (LMHP):
Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT). - Registered Nurse (RN).
- Licensed Practical Nurse (LPN).
- One year of experience with Medicaid or CCC Plus populations (Required).
- Two years of case management experience (Required).
Base pay range: $66,830.40 – $111,384. Additional compensation may include shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
