Position Summary

The Family Care Specialist provides skill building/case management services to tier 3 youth referred by the identified school under the Medicaid Rehabilitation Option (MRO) as well as Department of Children Services (DCS) standards of education to families in order to prevent removal of children from their families and/or to reunify children with their families. The Family Care Specialist will provide skills training to students, parents, and staff to assist in the development of skills that address mental health needs, decrease stigma, and behaviors that interfere with academic performance, cause conflict to peers, and put students at risk for a more restrictive academic placement.

Services will be provided year‑round at the school site, youth’s home during breaks or after school, and/or in the community setting.
* This position is a .75 FTE*

Essential Duties And Responsibilities
  • Provide individual or group skills training services to identified tier 3 youth referred from the school.
  • Enter progress notes/DCS documentation, complete State Registration and CANSA documents, maintain schedule in EPIC, and complete billing documentation within the timeline identified by policy.
  • Develop and maintain positive relationships and provide collaboration and advocacy for focused driven youth and family outcomes with both internal and external customers.
  • Keep current with all required trainings and certifications of both the DCS and the Family Health Center.
  • Meet the expectations of the Family Health Center, school, and DCS in all aspects of the role.
  • Provide case management services as allowed by MRO guidelines including internal assessments, CANSA (Child and Adolescent Needs and Strengths Assessments) and resourcing as needs of families arise.
  • Attend school functions, youth meetings (IEP, Wraparound, etc.), and trainings as identified by the school to assist in meeting the school and families needs.
Performance Requirements
  • Mental:
    Good judgment is required. Ability to communicate clearly both verbally and in writing with the treatment team as well as outside agencies. Must be able to engage in cooperative relationships.
  • Physical:
    Must pass physical exam. Capable of driving to outreach offices and family’s homes. Must be able to handle extremely active children.
  • Skills & Abilities:
    Sensitive and flexible when dealing with adults, adolescents, and children with a wide range of affective and behavioral disturbances. Demonstrate knowledge of and identification of needs, symptoms, and behaviors of individuals served. Manage time and workload, work independently as well as with a team, and adapt in various situations.
  • Working Conditions:

    Clean, well‑lighted, heated, and ventilated office. Will also work in the family’s home, other community agencies, and provide transportation in their own vehicles at times.
Minimum Qualifications (Knowledge, Skills, And Abilities)

Education: High School Diploma or G.E.D. Family Care Specialist must be 21 years or older for insurance purposes.

Experience: No specific requirements necessary. However, preference given to candidate with experience with youth and families. Will hire with experience if candidate has experience with children with complex needs.
