Community Health Worker - Healthy Neighbor Program, Fluent Spanish

Schedule:

Monday - Friday, 8:30-5:00pm, with potential night or weekend events. Locations:
Split time between home office Atlantic Avenue Camden, Community Events, and Client Home Visits.

The Community Health Worker (CHW) will be Healthy Neighbor’s frontline presence in the community. The CHW will work to improve patient health by providing social support, advocacy, and resource navigation; conduct health and wellbeing check-ins at clinics, community sites, patient homes, and remotely by virtual visits and phone calls; connect patients with follow-up clinic appointments and social resources; reinforce diabetes and hypertension health education;

and collaborate with patients to identify barriers to chronic disease management and work toward solutions. The CHW will build strong relationships with patients and their families.

• Perform needs assessments and enroll patients in Healthy Neighbor by meeting with prospective patients in-clinic, at community sites, and through remote outreach.
• Assist patients with their medical appointments – making appointments, sending reminders, coordinating transportation and supporting patients as needed.
• Refer patients to social services and community resources and follow up to ensure patient access to these supportive resources.
• Provide patients with health education and coaching to help them manage their diabetes and/or hypertension.
• Identify opportunities to improve Healthy Neighbor and work with the care team to implement changes.
• Establish and maintain community partnerships that support Healthy Neighbor and identify opportunities to improve outcomes; collaborate with the care team to implement changes.

• Required Experience:
  
  Demonstrated interest in community, social justice, public health and/or local economies.
• Ability to build relationships with people of diverse backgrounds, abilities, opinions, and perceptions; demonstrate equity and respect for all.
• Ability to travel to multiple work sites daily, using your own transportation. Valid driver’s license required if driving own vehicle.
• Available some evenings and weekends, depending on patient availability.
• Bi-lingual (Spanish) strongly preferred.
• Prior experience as a community health worker (CHW) or community-based patient advocacy, engagement, or resource navigation preferred.

High school diploma or GED.

Community Health Worker certification preferred. Lay navigation training certification will be considered as an alternative to CHW certification.
