Health-Related Social Screener

Cayuga Counseling Services, Inc. seeks a Health-Related Social Needs Screener (HRSN). This position is fully remote, part-time position and is expected to work a minimum of 15 hours per week. The HRSN Screener is responsible for conducting screenings for Medicaid members to determine eligibility under the Medicaid 1115 waiver program. The Screener also provides Navigation and Referrals to programs within the network.

• Conduct outreach referrals per program guidelines.
• Screen members for Social Needs and complete Eligibility Assessments.
• Navigate eligible members to programs within the network.
• Maintain strict compliance with HIPPA and all federal, state, and agency confidentiality regulations, policies and procedures.
• Document all services and outreach per program policies and procedures.
• Attend all required trainings.

• Experience with working with Medicaid or vulnerable populations preferred.
• Strong computer skills, including experience with Microsoft Office and electronic documentation systems.
• Knowledge of social services and community resources.
• Strong documentation and organizational skills.
• The ability to work independently.
