The Maryland Primary Care Program (MDPCP AHEAD) Community Health Worker will work closely with Primary Care providers and social services agencies in the state of Maryland to provide short term care coordination and connection to community or state resources. Will provide education and provision of coordination of care and services. Our Community Health Workers seek to develop, implement, and deliver extensive care coordination within our medical group and affiliated partners in a patient centered manner.
They interact and collaborate with interdisciplinary care teams, population health teams, networked facility partners, physicians, and others involved in meeting the patient’s needs.
- Coordinate care among individuals, communities, and health and social service systems
- Conduct outreach, through telephonic, office visits, and home visits
- Provide culturally appropriate health and informational resources
- Oversee case management and resource access
- Advocate for individuals and communities regarding housing, food security, and other social determinants of health needs
- Provide coaching and social support, such as nutrition training, recovery services for mental health and substance use disorders, and chronic disease management and prevention
- Organize and/or provide services, such as basic health screenings and first aid
- Increase individual and community participation in assessments and education to improve health and well-being
- High school diploma or GED; at least 1 year of experience directly related to the duties and responsibilities specified
- Possession of a valid Community Support Worker (CSW) certification preferred
- Possess planning, organizing, conflict resolution, negotiating and interpersonal skills
- Proficient with Microsoft Office applications including Word, Excel, and Power Point
- Ability to function efficiently in a virtual environment and meet defined goals
- Proficiency in multiple languages, preferred
The salary range for this role is $50,000 to $55,000 in base pay and exclusive of any bonuses or benefits (medical, dental, vision, life, and pet insurance, 401K, paid time off, and other wellness programs). This role is also eligible for an annual bonus targeted at 10%. The base pay offered will be determined based on relevant factors such as experience, education, and geographic location.
