Avail Health & Behavioral Solutions is seeking a motivated and compassionate Community Health Worker (CHW) to support individuals and families in navigating healthcare systems, accessing community resources, and improving overall health outcomes.

The CHW will serve as a bridge between clients, providers, and community services to reduce barriers to care and promote self-sufficiency.

This is a field-based role requiring strong communication, organization, and advocacy skills.

• Conduct outreach and engage individuals in the community
• Perform needs assessments and develop action plans
• Assist clients with accessing medical, behavioral health, housing, food, and transportation resources
• Support care coordination with physicians, case managers, and service providers
• Provide education on chronic disease management, medication adherence, and preventive health
• Assist clients with scheduling appointments and completing applications
• Maintain accurate documentation and case notes
• Participate in interdisciplinary team meetings
• Track progress toward client goals and outcomes

• Experience working with underserved populations
• Knowledge of housing navigation, employment resources, or re-entry support
• Bilingual a plus
• Experience with electronic documentation systems
