What to Expect (Job Responsibilities)
  • Provide direct supervision and oversight for a localized, multi-disciplinary team supporting Innovations members with mental health, substance use, and intellectual/developmental disabilities
  • Ensure staff utilize workflows to complete comprehensive assessments and care plans as members' needs change
  • Establish a team-based, person-centered approach to coordination of care that effectively manages members' health needs
  • Monitor teams to ensure integration of care through established multi-disciplinary processes
  • Translate knowledge of evidenced-based practices into specific processes and workflows for staff to enhance care coordination
What is Required (Qualifications)
  • Bachelor’s degree or licensure as an RN, with five (5) years of experience in care management, case management, or care coordination for individuals with I/DD or TBI; or a Master’s degree in a human services field or licensure as an RN, with three (3) years of relevant experience
  • Certification required if qualifying as a Registered Nurse
  • Valid driver’s license
  • Residency within North Carolina
  • Ability to travel within the catchment area as required

  • Work experience focused on Innovations Waiver
  • A minimum of two (2) years of experience supervising a team
