SPECIALTY CARE MANAGER
Catholic Charities of the Diocese of Rochester
Location: Rochester, NY
Schedule: Monday‑Friday
Employment Type: Full‑time
Salary: $25.00/hr.
General DescriptionProvide intake and ongoing care‑management services to individuals within Monroe, Finger Lakes, or Southern Tier Regional Counties diagnosed with one or more chronic illnesses. Effectively supports ongoing, collaborative communication among all program staff and ensures a trauma‑informed care environment.
Essential Duties and Responsibilities- Serve as contact for referrals and conduct necessary screening and/or follow‑up for client intake in applicable program(s).
- Provide client‑guided care‑management services to caseload assigned, maintaining accurate and verifiable documentation of all services rendered.
- Using a person‑centered approach, complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by DOH standards and Health Home contractors.
- Promote and practice cultural competency toward clients through respect and understanding.
- Support diversity by encouraging the strengths of everyone served.
- Provide core services for each client assigned monthly and as needed, ensuring all care plan goals are satisfied to client satisfaction.
- Maintain accessibility for clients during the workday with a team approach.
- Assist clients in acquiring and maintaining public benefits (e.g., Medicaid, SNAP, Social Security).
- Provide linkage/referrals and coordinate resources/support for medical care, mental health, social, recreational, and housing/basic needs.
- Remain current in the field, supporting and implementing best practice service models within the program.
- Make independent decisions regarding immediate risk factors and provide crisis intervention and follow‑up as needed.
- Demonstrate flexibility to be available to clients in their homes/communities, including outside normal business hours.
- Provide on‑call availability via cell phone to Health Home program as needed.
- Serve as client advocate.
- Facilitate the development of a Care Team to deliver individualized, strength‑based Plans of Care through regular case conferences and case reviews.
- Participate in staff meetings and other Care Coordination department activities.
- Ensure compliance with all applicable local, state, federal and Health Home Network regulations and agency policies.
- Participate in mandatory ongoing training and professional development offered.
- Represent Catholic Charities Family and Community Services on appropriate boards and committees as assigned by the Director.
- Participate in relevant agency meetings and/or trainings.
- Pursue development of professional competencies through reading, supervision, in‑service training, and attendance at conferences and workshops.
- Support the Agency’s Mission, Vision, Values and Strategic Plan, and core competency initiatives (collaborative & community‑focused, innovative & entrepreneurial, culturally competent & diverse).
- Other duties as assigned.
- Education: Bachelor’s Degree.
- Credentials: Valid NY State Driver’s License with an acceptable driver’s record. Bilingual (English/Spanish) required for some positions; additional compensation offered if bilingual skills are used.
- Experience: At least 2–4 years of practical experience working with individuals with disabilities or chronic illnesses.
- Combination of education and experience may be substituted at the discretion of the Director.
