Per Diem Inpatient Pharmacist

IdealCandidate
The Ideal candidate will hold a Bachelor of Science degree in Pharmacy or a Pharm.
D. degree from an accredited college of pharmacy, with a professional doctorate preferred. At least one (1) year of inpatient pharmacy experience is preferred. The candidate will demonstrate strong clinical knowledge, attention to detail, and the ability to work effectively within a multidisciplinary healthcare team.
Will be Per Diem and on an as needed basis.

Core pharmaceutical services include, but are not limited to, the following:
- Review, clarify, and process medication orders accurately and in a timely manner
- Assess appropriateness of ordered medications based on disease state, body surface area (BSA), age, height, weight, laboratory values, and renal function
- Utilize computer systems and clinical resources to support pharmaceutical care, including:
- Medication order entry and verification
- Medication distribution and checking
- Medication reconciliation
- Perform and document therapeutic interventions as appropriate
- Actively participate in event reporting through UHC Safety Intelligence
Responsibilities include, but are not limited to, the following:
- Supervise immediate technical staff, including:
- Assignment completion
- Monitoring lunch breaks
- Ensuring adherence to institutional and departmental policies (e.g., sterile and non-sterile compounding)
- Ensure timely completion of all tasks to maintain appropriate workflow and optimal patient care
- Maintain a clean and organized work area at all times
- Monitor inventory and supplies; notify appropriate personnel of shortages or outages
- Oversee sterile and non-sterile compounding activities
- Complete all assigned Quality Assurance and productivity reports, including documentation and follow-up
- Research and monitor operational issues, initiate corrective actions, and follow through to resolution
- Provide consistent input and suggestions for process improvement
- Maintain compliance with all institutional and divisional requirements, including:
- Licensure
- Mandatory education and testing (e.g., EEE, Infection Control, age-related competency, Code Blue, BLS certification, TB testing, IV process validation)
- Develop, implement, and/or support special projects across the institution
Clinical and patient care responsibilities include, but are not limited to:
- Actively participate in clinical initiatives such as:
- Discharge counseling
- Medication reconciliation
- Renal dose adjustments
- Conduct and document patient education as appropriate
- Document patient-specific information in the electronic medical record
Bachelor of Science degree in Pharmacy or Pharm.
D. degree from an accredited college of pharmacy.
Doctorate (Professional)
CertificationAll of the following:
- Texas State Board of Pharmacy Pharmacist license within 90 days of job entry.
- American Heart Association (AHA) Basic Life Support (BLS) certification within 30 days of job entry.
- American Heart Association (AHA) Advanced Cardiac Life Support (ACLS) certification within 90 days of job entry, if working FT in one of following departments:
Pharmacy Inpatient (710348), Pharmacy ATC ACB (710355), Pharmacy-West Houston (710657) Pharmacy-The Woodlands (710480)
- ACPE Sterile Products Certification
- ACLS/BCLS Certification
None.
Preferred ExperiencePrefer 1 year of experience in a hospital pharmacy or ASHP PGY1 Residency training.
