A health service provider is seeking an experienced Medical Case Manager to support members in healthcare programs in the LaSalle/Peru area. The ideal candidate will have a valid clinical care license (RN, LPC, or LMSW) and over 3 years of managed care experience. Responsibilities include care coordination, health education, and assessing member needs while ensuring effective communication and documentation. This is an ongoing W2 contract position with travel involved.

Flexible remote work is also an option.
