Job Description
The Field Care Manager Nurse 2 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members. The Work assignments are varied and frequently require interpretation and independent determination of appropriate courses of action.
Must reside in Michigan with the ability to drive to Wayne or Macomb Counties. This position will be based from a home office and will travel 75% of the time to an assigned area in Wayne or Macomb County to conduct in‑home visits with Medicare/Medicaid members. The Field Care Manager employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho‑social health issues.
PositionResponsibilities
- Provides clinical support and guidance, particularly for members with medical complexity; helps develop and coordinate care plans ensuring that patients receive appropriate services to manage their health needs effectively.
- Addresses barriers to health care and advocates for optimal member outcomes.
- Reviews, assesses, and completes medical complexity attestations and clinical oversights.
- Ensures members receive services in the least restrictive setting to achieve and/or maintain optimal well‑being by assessing their care needs.
- Develops and modifies Individual Care Plans and involves applicable members of the care team in care planning (informal caregiver, coach, PCP, etc.).
- Supports members and/or caregivers using an interdisciplinary approach to access social, housing, educational, and other services, regardless of funding sources, to meet their needs.
- Primary point of contact for the Interdisciplinary Care Team (ICT) and is responsible for coordinating with the member, ICT participants, and outside resources to ensure the member’s needs are met.
- Registered Nurse, Nurse Practitioner, or Clinical Nurse Specialist with a minimum of 2 years experience in health care and/or case management.
- Active Michigan Registered Nurse (RN) license with no disciplinary action.
- Must reside in the state of Michigan.
- Ability to travel to homes and community settings for face‑to‑face assessments.
- Experience working with the adult population and disease management.
- Knowledge of community health and social service agencies and additional community resources.
- Exceptional communication and interpersonal skills with the ability to quickly build rapport.
- Ability to work with minimal supervision within the role and scope.
- Proficiency with electronic information applications & software programs, including electronic medical records.
- Excellent keyboard and web navigation skills.
- Intermediate to advanced computer skills and experience with Microsoft Word, Outlook, and Excel.
- Ability to work full‑time (40 hours minimum) Mon‑Fri.
- Valid driver’s license, car insurance, and proof of personal vehicle liability insurance with limits of 100,000/300,000/100,000.
- Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work.
- Must have accessibility to high‑speed DSL or cable modem for a home office (satellite internet service is NOT allowed for this role); recommended speed for optimal performance from client At Home systems is 25 Mbps or 10 Mbps.
- Role is part of client At Home’s Tuberculosis (TB) screening program; if selected, you will be required to be screened for TB.
- 75% travel is required in this position.
- BSN.
- Experience with in‑home assessment and care coordination.
- Experience with health promotion, coaching, and wellness.
- Experience with Medicaid long‑term care.
- Previous managed care experience.
- Bilingual in Spanish, Arabic or Chaldean Neo‑Aramaic.
