Family Care Coordinator - VMBH Fallstaff

Overview

Salary: $52,500. Catholic Charities of Baltimore, Villa Maria Behavioral Health is currently seeking a Family Care Coordinator (FCC). The work schedule will vary between Monday-Friday 11:00 AM – 7:00 PM. Schedule is flexible and hybrid between in-office, home visitations and remote work. Villa Maria Behavioral Health Clinics provide mental and behavioral health services to children, adults, and families across Maryland. Services include individual counseling, family counseling, psychiatric evaluation, social and coping skill development, addiction counseling and treatment, and medication management.

• Conducts a family needs assessment and aligns with the family to develop a care plan or an IRP (Individual Rehabilitation Plan).
• Uses motivational interviewing with the youth and family as they pursue goals.
• Coordinates meetings and communicates with outside providers, especially schools, both verbally and in writing.
• Utilizes knowledge of local family supports and helps the family access supports for basic needs, medical, and transportation.
• Maintains written records of work, including the family assessment and ongoing meetings with family members or providers. Documentation in the electronic medical record such as contact notes and treatment plans.
• Provides updates on family progress as needed, including quarterly reporting.
• Performs other duties as assigned.

• High School/GED and 5 years of work experience with emotionally disturbed children and adolescents and their families.
• OR Bachelor’s degree in social work, psychology, or related field and one year of work experience with emotionally disturbed children and adolescents and their families.

• Shows reliability by being punctual to meetings with family members and outside agencies.
• Excellent relational skills with courteous demeanor toward families and providers.
• Ability to develop a schedule in collaboration with their supervisor to meet the needs of an assigned caseload.
• Ability to remain calm in stressful situations.
• Ability to engage families who are experiencing setbacks and discouragement.
• Ability to work with small groups of children or family members.
• Excellent communication skills including active listening.
• Service-oriented and able to work independently.
• Ability to utilize computer systems and software required to perform position functions, including basic Windows PC, web browser usage, and Microsoft Outlook. Knowledge of Word, Excel, PowerPoint, Skype, One Drive desired. FCC will be expected to use the Electronic Medical Record for documentation daily.

• Moving about to accomplish tasks or move between sites.
• Communicating with others to exchange information.
• Hearing and vision abilities within normal range (corrected) to drive safely and observe and communicate with others.
• Driving (agency and/or personal vehicle) is required. Driver must be at least 21 years old, with a minimum of 2 years of driving experience, and a valid driver’s license. No state-issued restrictions that impede operation; no more than three (3) points on driving record.
