Cape Cod Health Care Facility seeks a full time Outpatient Psychiatric Nurse Practitioner to serve patients who are homeless, or at-risk of homelessness, many with multiple conditions including mental health and substance abuse issues, in a compassionate, collaborative, educational and caring environment.

• Provide psychiatric mental health assessment, including description of presenting problem, psychiatric history, family history of mental health problems, relevant medical history, substance abuse history, risk assessment, etc.
• Provide individualized and culturally sensitive medication management applying evidence-based or best-practice standards where applicable.
• Provide clear documentation of all services and interventions.
• Collaborate with patients and care teams to support treatment goals.
