Job Summary
The Field Care Manager, Behavioral Health, is responsible for assessing and supporting members to achieve or maintain their highest level of wellness. This role involves conducting in‑home and community‑based assessments, developing care plans, coordinating interdisciplinary care, and ensuring members receive appropriate services in the least restrictive setting. The position requires independent judgment, strong clinical skills, and the ability to collaborate with multiple care partners.
This is a home‑based role with required travel to members' homes within Wayne or Macomb County.
Responsibilities
- Manage a caseload and conduct member assessments in home, community, and telephonic settings.
- Provide clinical guidance and collaborate with interdisciplinary care team participants to support members with complex needs.
- Develop, update, and coordinate individualized care plans to ensure members receive appropriate services.
- Identify and address barriers to care and advocate for optimal member outcomes.
- Complete medical attestations and provide clinical oversight as required.
- Ensure members receive services in the least restrictive environment.
- Facilitate access to social, housing, educational, and community resources.
- Serve as the primary point of contact for the interdisciplinary care team, coordinating with members, providers, caregivers, and community agencies.
Required Qualifications
- Bachelor’s degree in Social Work with active state licensure.
- Minimum 2 years of post‑degree clinical experience in a behavioral health setting.
- Must reside in Michigan with ability to travel to Wayne or Macomb Counties.
- Experience working with adult populations and disease management.
- Knowledge of community health, social service systems, and related resources.
- Strong communication and interpersonal skills with the ability to build rapport quickly.
- Ability to work independently with minimal supervision.
- Proficiency with electronic medical records and computer systems including Microsoft Word, Outlook, and Excel.
- Valid driver’s license, automobile insurance, and access to a reliable vehicle.
- Ability to maintain a private, secure home office workspace.
- High‑speed cable or DSL internet (satellite connections not permitted).
- Ability to meet health and screening requirements for direct patient‑facing roles.
Preferred Qualifications
- Master’s degree in Social Work (LMSW/LCSW).
- Experience with in‑home assessments or care coordination.
- Experience in health promotion, coaching, or wellness programs.
- Experience with Medicaid Long‑Term Care programs.
- Managed care experience.
- Training or certification in Motivational Interviewing.
