The Child Center of NY is seeking a dedicated and experienced Youth ACT Team Leader to oversee its Youth Assertive Community Treatment (ACT) program. The Youth ACT Team Leader plays a critical role in ensuring that the program provides intensive, community-based behavioral health services to youth (ages 10–21) with serious emotional disturbance (SED) and complex behavioral health needs who are at risk of psychiatric hospitalization or out-of-home placement.
The Team Leader supervises an interdisciplinary team, ensures compliance with OMH guidelines, and oversees clinical quality, service delivery, and staff development.
- Provide clinical and administrative leadership for the Youth ACT team
- Oversee daily operations of the program and ensure compliance with NYS Office of Mental Health (OMH) Youth ACT guidelines
- Supervise and support team members including licensed clinicians, nurses, peer specialists, care managers, and psychiatric providers
- Facilitate interdisciplinary team meetings, case reviews, and treatment planning sessions
- Assign and monitor caseloads, ensure timely access to services, and address barriers to care
- Serve as primary liaison with OMH, community partners, schools, and other systems of care
- Provide direct clinical services to a small caseload, including crisis response and clinical assessments
- Ensure fidelity to the ACT model, including use of evidence-based practices and trauma-informed care
- Maintain program documentation and ensure data collection for reporting, quality assurance, and evaluation
- Lead staff recruitment, onboarding, performance reviews, and ongoing professional development
- Promote family engagement, youth voice, and culturally competent service delivery
- Participate in on-call rotation to support crisis coverage
- Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), Licensed Psychologist, Psychiatric Nurse Practitioner (PMHNP), or Psychiatrist licensed in New York State
- Minimum of 3 years of clinical experience with youth with serious emotional disturbance (SED) or co-occurring disorders
- Minimum of 2 years of supervisory or program management experience in a behavioral health or child welfare setting
- Strong knowledge of ACT principles, OMH regulations, and community-based service models
- Excellent leadership, organizational, and communication skills
- Proficiency with electronic health records and Microsoft Office Suite
- Valid NYS driver’s license and ability to travel for community-based visits
- Prior experience working in an ACT, Home-Based Crisis Intervention (HBCI), Health Home, or similar mobile treatment program.
