Overview
Epitec is seeking an experienced Medical Case Manager to join our Health Care Provider client's team. As a Medical Case Manager, the candidate will play a pivotal role in supporting members enrolled in healthcare programs in the LaSalle/Peru area.
- This is an ongoing W2 contract
- This position involves working from home and traveling to residences
- Provide medical management / care coordination for members enrolled in government healthcare programs, acting as a key clinical resource and advocate.
- Deliver health education and wellness support to members to improve outcomes, promote self-management, and reinforce care plan goals.
- Assess member needs (clinical, psychosocial, barriers to care) and help develop, implement, and update individualized care plans.
- Coordinate care with physicians and care teams, and connect members to community resources and external vendors as appropriate.
- Gather, analyze, and maintain clinical/medical data to support regulatory reporting and internal tracking.
- Document interventions and outcomes accurately and timely using PC-based tools and web applications.
- Represent the organization to members with professional, empathetic communication, including discussing medical needs and next steps.
- Travel within the assigned territory for in-home/community-based member visits; complete documentation/notes from home when not in the field.
- One of the following licenses/credentials:
- Registered Nurse (RN) with 3+ years of direct clinical care in a clinical setting, OR
- Licensed Professional Counselor (LPC), OR
- Licensed Master Social Worker (LMSW) with 2+ years of clinical practice toward LPC/LMSW licensure.
- Current, valid, unrestricted license in the state of operations (or reciprocity). If compact RN relocating, obtain active unrestricted state licensure within 90 days of hire.
- 3+ years wellness or managed care experience presenting clinical issues with members and/or physicians.
- Strong verbal and written communication skills (member-facing + internal/external partner communication).
- Analytical experience, including medical data analysis and supporting regulatory reporting.
- Valid driver’s license, reliable transportation, and insurance; ability/willingness to travel in assigned territory.
- PC proficiency (Word, Excel, PowerPoint), databases, and web-based applications.
