We are seeking a Field Nurse Case Manager to cover Fort Myers and surrounding communities, including Naples, Lehigh Acres, Cape Coral, Punta Gorda, and Port Charlotte. A Field Case Manager role is a home-based position with travel up to 2 hours one-way (4 hours round trip) to doctors’ offices, hospitals, and other locations.

This position is responsible for the medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors progress and reports results back to the employer and insurance carriers.

• Provide comprehensive medical case management for workers’ compensation cases, including assessment, planning, coordination, implementation, and evaluation.
• Collaborate with insurance carriers, medical care providers, attorneys, employers, and employees; monitor progress and report results.
• Travel to doctors’ offices, hospitals, and other sites within the assigned geographic area.
• Coordinate care to achieve optimal patient outcomes and promote safe, timely return-to-work when applicable.

• Current, unencumbered Registered Nurse (RN) license in Florida.
• Experience in case management of workers’ compensation cases preferred.
• Recommended certifications (CCM, CRRN, CDMS, CRC) preferred; REQUIRED within two years of hire date.
• Skills:
  
  organization, timeline adherence, PC/technology proficiency, written/verbal/interpersonal communication, autonomous work ability, patient education.
• Valid Driver’s License and good driving history.
• Bilingual Spanish preferred.
