As a Licensed Behavioral Health Care Manager (BHCM) at Heritage Health Network, you will play a critical role in addressing the behavioral health needs of our members through the Enhanced Care Management (ECM) program. This role focuses on delivering culturally sensitive and empathetic care while managing behavioral health services, coordinating with interdisciplinary teams, and ensuring the integration of behavioral health with other medical and social services.
This role combines remote work, in-office responsibilities, and field engagements, with work arrangements tailored to meet program and member needs.
Key Responsibilities- Conduct comprehensive assessments to identify behavioral health/psychosocial needs and develop individualized care plans.
- Oversee clinical operations and manage ECM team members, ensuring staff compliance with regulatory standards.
- Provide strategic leadership for clinical services, guiding clinical managers and interdisciplinary care teams.
- Deliver individual, family, and group therapy sessions, as well as workshops and support groups for members and their families.
- Maintain accurate and confidential records of all encounters in electronic health records (EHR), adhering to confidentiality standards.
- Engage in community outreach, providing presentations and participating in health education initiatives.
- Serve as a key liaison between members, their families, and external behavioral health resources.
- Provide ongoing management of behavioral health conditions, including crisis intervention, counseling, and regular progress evaluations.
- Education: Must have a Master's degree and current licensure (LCSW, LMFT, LPCC, or Psychologist) in the state of California.
- Experience: Minimum 1 year of experience providing direct behavioral health care services.
- Skills: Strong interpersonal, communication, and documentation skills; ability to work effectively with diverse populations.
- Knowledge: In-depth knowledge of behavioral health care practices, Medi-Cal regulations, and various behavioral health interventions (e.g., for Depression, Anxiety, Trauma, SUD).
- Commitment: Deep understanding of health disparities affecting vulnerable populations and a commitment to providing culturally competent care.
- Mobility: Willingness to travel within Los Angeles county for home visits and community engagement.
