Support clients to implement NDIS plans by coordinating access to informal, community, mainstream funded supports and liaising directly with the NDIA.

About the Role

As a Support Coordinator, you’ll help clients access services under the National Disability Insurance Scheme (NDIS). You’ll assist with implementing plans, connecting clients to informal, mainstream, community, and funded supports, and liaising with the NDIA.

You’ll play a vital role in improving quality of life, advocating for autonomy, dignity, and respect, and fostering a nurturing environment.

Key Responsibilities
  • Provide information on services and supports best suited to the client
  • Assist clients in resolving points of crisis and service delivery issues
  • Help clients understand their NDIS funding and plan their budgets
  • Support clients to work towards their goals as outlined in their NDIS plan
  • Assist with plan reviews and requests for additional funding
  • Provide welfare support to MSWA clients as required
What We’re Looking For
  • Experience working in community services within disability
  • Experience supporting clients with NDIS plans
  • Effective communication and time management skills
  • Ability to work independently and as part of a team
  • Basic computer skills
  • NDIS Worker Screening Check and Orientation Certificate (or willingness to obtain)
  • First Aid & CPR Certificate
  • A current ‘A’ Class Driver’s Licence
