Clinician, Home Based Foster Care
Social Workers and Mental Health Professionals with Limited Permit are eligible for a $2500 retention bonus!
Licensed Social Workers and Mental Health Professionals are eligible for a $7500 retention bonus!
As a Clinician, you will coordinate and complete client assessments, in collaboration with the child, family, legal guardian, law guardian, psychiatrist, nurse, psychologist, educators, childcare staff, and other providers to prepare, implement and monitor a comprehensive, permanency-focused case plan for children in the Home-Based Foster Care Setting. You will report to the Clinical Supervisor and will work onsite and in the community.
Responsibilities- Assess children/Families at intake to complete a comprehensive mental health assessment to create a diagnosis and comprehensive treatment plan for recommended service needs.
- Meet and communicate with biological families, guardians, foster families, and case workers.
- Provide therapeutic services to identified children/Families based on treatment plan recommendations.
- Work with an interdisciplinary team to monitor ongoing progress of children and families and update treatment plans.
- Work with referring and collateral agencies, prepare reports, and correspondence.
- Maintain Clinical case records observing Agency, OMH, and other state mandates.
- Diligently plan for discharge/permanency, at the time of admission.
- Communicate with your client and treatment team.
- Master's degree in Social Work or related mental health field required.
- One year of related experience required.
- Professional licensure required, Limited permits accepted: LMSW with 3 years experience, LMHC‑D, LMFT‑D, LCAT, LCSW.
- Valid NYS driver's license required.
