Overview
The Home Based Primary Care Dietitian coordinates nutritional care activities for all assigned Veterans, including those with complex medical and nutritional needs. The clinical dietitian supports the facility's mission and coordinates nutritional care with other health care providers, the veteran, the Veteran's caregiver and community. This position is located at the Hamilton CBOC and is a mobile position requiring visits to Veterans homes.
RequirementsA valid state-issued driver license is required.
DutiesEmployees in this assignment serve as a full performance Dietitian (Clinical) providing Home Based Primary Care (HBPC) within the Clinical Section of Nutrition and Food Service serving as the medical nutrition expert.
