The Engagement Coordinator is responsible for initiating and maintaining patient engagement with Strive’s comprehensive care program. This role will often be the first to interact with a patient, introducing them to Strive’s services. It includes placing outbound calls and, as needed, meeting with patients in-person to connect them with the appropriate Strive resources and schedule visits. Additionally, the individual will leverage internal and external data platforms and local market resources to identify and update patient contact information.
The role requires clear communication, enthusiasm, and a passion for connecting directly with patients. It reports to the Lead Engagement Coordinator.
- Proactively outreach to patients via phone to educate them on the care services available and connect them with the appropriate Strive resources.
- Meet or exceed performance targets defined as “completed first visits” on a daily basis.
- Research and update patient information, leveraging internal and external data platforms, including EHRs and local market resources.
- Build key internal relationships to elevate patient support needs.
- Lead with empathy, gain trust, and develop an initial understanding of individual patient needs.
- Support the development of long‑term patient relationships with Strive through aligned messaging and communication that conveys knowledge of the patient’s health condition and personal history.
- As necessary, visit with patients in‑person at local physician offices or patient homes with the goal of scheduling a future visit; travel may be required to key markets within your territory.
- 2+ years combined of related education, experience, or certification.
- CPR Certification required.
