Job Summary
The Patient Care Assistant II (PCA II) functions as a member of the patient care team, working under the direction of the Registered Nurse (RN) to consistently perform all aspects of the PCA role. Responsibilities include supporting the RN with clinical tasks such as blood glucose monitoring, completing EKGs, applying simple dressings, utilizing aseptic technique, and responding to medical emergency situations using Heartsaver CPR and AED skills.
The PCA II also attends to the needs of patients and family members, promotes the Patient and Family Centered Care Model, and contributes to efficient unit operation.
- Provision of a Safe Environment
- Anticipate and identify patient safety risks in cooperation with the licensed nurse, taking actions to mitigate risks (e.g., proper bed height, use of side rails, alarms, call lights).
- Perform routine patient room rounds and assist with ambulation as indicated.
- Follow restraint policies under RN supervision and maintain 1:1 monitoring for patients at risk for injury.
- Adhere to infection control and safety procedures and use proper body mechanics to prevent personal injury.
- Organizational Ethics
- Maintain an environment reflecting the organization’s mission and values.
- Respect and support a culturally diverse staff and patient population, providing nondiscriminatory care.
- Respect confidentiality and privacy, resolve interpersonal conflicts cooperatively, and adhere to dress code including visible hospital .
- Management of Resources
- Organize time, prioritize patient care needs, and collaborate with other PCAs.
- Assist co‑workers and adapt to changing unit needs (floating, adjusting assignments).
- Coordinate break periods with Charge Nurse/Nursing Supervisor and maintain unit visibility.
- Management of Information
- Understand and follow policies, procedures, and work rules.
- Know emergency codes and how to call a code.
- Use hospital e‑mail, read meeting minutes, attend staff meetings, and communicate patient status to the RN promptly.
- Accurately collect and record patient data (vitals, intake/output, blood sugar, weights, restraints) to ensure a complete patient record.
- Collaborative Interdisciplinary Care
- Maintain a teamwork approach to patient care.
- Patient/Family Education
- Provide nonclinical information and orient patients/families to the hospital and unit, distribute mail/flowers, organize personal care items, and run errands as assigned.
- Reinforce education provided by the nurse and give basic instructions (call light, positioning, transfer, walking, toileting, feeding).
- Competency
- Adhere to practice standards, policies, protocols, and regulations.
- Maintain competence in technical skills (external monitors, CPM machine, EKGs, blood glucose, dressings, CPR/AED).
- Assist in orienting new PCAs and serve as a resource for float staff.
- Quality Improvement
- Participate in hospital‑wide and unit‑based SEAQ programs.
- Communicate quality concerns to the Nursing Director, Clinical Leader, or Nursing Supervisor.
- Patient Care
- Assist with admission, discharge, and transfer of patients; prepare rooms and ensure correct identification.
- Transport patients and belongings, provide personal care (bathing, skin care, grooming, perineal care, changes of linens).
- Provide basic skin care, turn and reposition patients, assist with elimination, and offer comfort measures.
- Carry out 1:1 monitoring for high‑risk patients.
- Perform simple dressing changes, assist with nursing procedures, set up meals, and record intake/output.
- Conduct EKGs and blood glucose monitoring per policy.
- Document patient information accurately and timely on flow sheets.
- Support end‑of‑life care and post‑mortem care; label and send specimens to lab.
- Track and secure patient belongings as per protocol.
- Respond to call lights and requests, maintain orderly patient rooms, restock supplies, and dispose of waste properly.
- Identify and resolve equipment issues, uphold safety inspections, and initiate emergency responses.
- Education
:
