The Senior Manager of Hospital Outreach will lead a team of Dialysis Transition Managers. These individuals are responsible for conducting outreach to hospitals in an effort to improve the transition of care experience for renal patients transitioning from the hospital to outpatient dialysis. This leader will drive change through others by helping to coach teammates in order to improve the quality of outreach and enable learnings and feedback from these key hospital stakeholders.

They will be required to work cross functionally to identify, prioritize, and communicate the opportunities that will have the greatest impact on patient and hospital experience.

The Senior Manager will work with a team of highly motivated and entrepreneurial individuals and is expected to: (1) effectively lead work streams with dependencies on many different business units, (2) collaborate well throughout the organization, and (3) communicate recommendations to senior executives.

• Leverages hypotheses-based approach to identify, validate and quantify opportunities
• Develops infrastructure around data, tools and processes to create sustained organizational capabilities
• Coaches and develops others to improve skills necessary to be successful in their role

  Goes deep when necessary and zooms out when appropriate
• Successfully manages relationships with senior leaders and peers and can influence a diverse group of stakeholders through effective communication and strong interpersonal skills
• Works closely with teammates in the clinics and develops long-standing relationships with operators and clinicians
• Allocates team resources to ensure all timelines and deliverables are met
• Creates and manages project plans that drive accountability for complex, cross-functional initiatives;
  Gets the right stuff done
• Up to 30% travel may be required at times; though typical travel is 5-10%

• Experience leading others
• Experience driving transformational change through teams from concept through implementation with tangible impact on operating/financial performance
• 5-10 years of total work experience in progressive roles
• Experience or degree in a healthcare related field strongly preferred

• Consistent outreach to discharge planners to provide education and build awareness of offerings
• Identify opportunities to grow as provider of choice through relationship building and addressing feedback
• Partner with internal field teammates to optimize the admission process and implement best demonstrated practices
• Assist with necessary communication between hospital, admission and clinic teammates to work through patient placements with extenuating circumstances
• Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers
• Develop comprehensive knowledge of local market
