As a RN Field Clinical Care Coordinator with United Health Group, you'll wear many hats and work in a variety of environments.

This is a Field Based position with a Home‑Based office.

If you are located in or within commutable driving distance to the Southeast Houston, TX area, you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.

Primary Responsibilities
  • Provide a complete continuum of quality care through close communication with members via in‑person or on‑phone interaction
  • Support members with condition education, medication reviews, and connections to resources such as Provider Services, Private Duty Nursing, DME, and supplies
  • Assist members with the transition from a care facility back to their home
  • This role involves approximately 75% travel, and you'll need to be comfortable dealing with a full range of in‑home and facility‑based environments
  • Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential
  • This function is for UHC C&S Only (with the exception of the Community Health Worker role)
  • Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well‑being of individuals, families, and communities
    • Generally, work is self‑directed and not prescribed
    • Works with less structured, more complex issues
    • Serves as a resource to others

Required Qualifications
  • Associate's degree (or higher)
  • Current unrestricted RN license
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management
  • Intermediate level of computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Valid driver's license, access to reliable transportation and the ability to travel up to 75% in an assigned region to visit Medicaid members in their homes and/or other settings
  • Reside in Houston, TX area
Preferred Qualifications
  • Case management experience including Certification in Case Management
  • Experience working with the needs of vulnerable populations who have chronic or complex bio‑psychosocial needs
  • Experience in Home & Community based on Long‑Term Care services delivery
  • Home care/field‑based case management
  • Bilingual skills
  • Reside in the SE Houston, TX area
