GENERAL DESCRIPTION
Under the supervision of the Family Engagement Manager, the Perinatal Home Visitor is responsible for the provision of services for pregnant women and new mothers and ensuring compliance with all related Performance Standards. The Perinatal Family Services Specialist partners with families to strengthen protective factors, support prenatal and postpartum well-being, and ensure a successful transition into Early Head Start or other appropriate early learning settings.
ESSENTIALFUNCTIONS
- Recruit and enroll eligible pregnant women into the program. Maintain full enrollment at all times.
- Coordinate health and pregnancy related trainings and seminars for pregnant women and new mothers.
- Work closely with entire family to ensure that the newborn child enters a safe and nurturing home.
- Coordinate socialization activities in accordance with the Head Start Performance Standards.
- Contact clients once a week and conduct at least two home visits per month with enrolled clients.
- Provide specialized prenatal and postpartum education and individualized coaching related to maternal well-being, healthcare systems, insurance enrollment (e.g., Medi-Cal), mental health awareness, and infant development.
- Coordinate continuity of services in partnership with Home Visitors to ensure seamless transition into Early Head Start center-based/home-based services or other community childcare settings.
- Attend parent, staff meetings and training as assigned.
- Serve as a liaison and promote effective relationships between the program and the community.
- Assist families in receiving emergency and other assistance services.
- Maintain and enhance professional competence through participation in professional organizations and other activities on a voluntary or assigned basis.
- Assume responsibility for parent involvement as appropriate.
- Maintain accurate, current and follow-up documentation on recruitment, enrollment, referrals, health and social services and other information related to the children and families.
- Coordinate statistics to ensure accurate completion of monthly reports.
- Work cooperatively with the Site Supervisor and all other staff to coordinate responsibilities and activities.
- Attend not less than 15 clock hours of professional development per year.

EXPERIENCE AND EDUCATIONA A or higher degree in the area of Maternal and Child Health, Human Development, or Child and Family Development. One year experience working directly with pregnant women, new mothers and infants; must have knowledge of medical resources within the general San Diego community; ability to work easily and effectively with parents from diverse ethnic and socioeconomic backgrounds; require flexible work hours (e.g. some evenings and weekend work is expected).
LANGUAGEBilingual English/Spanish preferred.
OTHER/SPECIALExcellent oral and written skills. Ability to pass a physical examination and tuberculin skin test. Fingerprint clearance and criminal record check required. Ability to conduct numerous visits to clientele’s homes at various locations in San Diego’s South County Area. Verification of current driver’s license and insurance required for personal car usage.
