Job Details
• Home health setting serving a primary care patient population,
• Day shift, 8:00 AM-5:00 PM, Monday through Friday with rotating weekends,
• 8-hour shifts, 5 days per week,
• Mileage between patient visits is reimbursed,
Job Requirements
• Current, active OT license (license required if applicable),
• Minimum 1 year of experience as an Occupational Therapist,
• Home health experience required,
• Current BLS certification required,
• Illinois state license required,
• Candidates must reside more than 50 miles from the facility to be eligible,
Additional Information
• Provide occupational therapy services to patients in their homes as part of a home-based primary care program,
• No local candidates within a 50-mile radius of the facility will be considered,
