• Conduct initial and ongoing occupational therapy evaluations in the home
• Assess patient functional status, including strength, endurance, coordination, communication, and self‑care abilities
• Evaluate the home environment for safety risks and barriers to independent living
• Develop, implement, and update individualized plans of care in collaboration with the Health Care Provider and RN
• Complete initial evaluations, treatment notes, and re‑evaluations at least every 30 days
• Complete comprehensive assessments and OASIS documentation accurately and on time
• Observe, document, and report patient progress and significant changes; obtain updated orders as needed
• Identify and recommend adaptive equipment and strategies to improve independence with activities of daily living (ADLs)
• Educate patients and caregivers on adaptive techniques and devices
• Provide supervision and support to Occupational Therapy Assistants (OTAs), as assigned
• Collaborate with physicians and interdisciplinary team members to deliver high‑quality care

• A compassionate, patient‑focused Occupational Therapist
• Comfortable working independently in a home health setting
• Organized, detail‑oriented, and timely with documentation

• Motivated by meaningful, one‑on‑one patient care

• Bachelor’s degree from an accredited Occupational Therapy program
• Active Occupational Therapist license in the state of practice
• Minimum of one (1) year of OT experience; home health experience preferred
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to complete home visits and document care accurately and timely
• Home health documentation experience preferred (Home Care Home Base a plus)
