Perkins Spine and Sports Medicine, LLC is a growing, patient-centered spine and pain management practice committed to whole-person healing. As our patient population continues to grow, we are expanding our behavioral health services to better support our geriatric and chronic pain patients with accessible, high-quality therapy services.

Position Overview

We are seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP), Licensed Psychologist or Licensed Independent Social Worker (LISW) to provide outpatient therapy services to our geriatric and pain management population.

Our behavioral health population is mid-acuity, primarily consisting of patients navigating chronic pain, adjustment disorders, depression, anxiety, and aging-related psychosocial stressors. Patients requiring high-acuity psychiatric care are referred to psychiatry for specialty management, allowing this role to focus on structured outpatient therapeutic intervention within an integrated and collaborative medical setting.

The role is primarily remote, with optional minimal in-office opportunities at our Dublin location.

Responsibilities
  • Provide individual psychotherapy to geriatric and chronic pain patients
  • Conduct behavioral health assessments and develop structured treatment plans
  • Collaborate closely with spine and pain providers regarding integrated care plans
  • Document encounters in a timely and compliant manner
  • Maintain professional licensure and regulatory compliance

Note:

Medication management may be included for PMHNP candidates depending on scope and mutual agreement.

Qualifications For PMHNP Applicants:
  • Active PMHNP license in the State of Ohio
  • DEA (if providing medication management)
  • Experience with geriatric, chronic pain, or medically complex patients preferred
  • Active Psychologist license in the State of Ohio
  • Experience with geriatric, chronic pain, or health psychology preferred
For LISW Applicants:
  • Active LISW license in the State of Ohio
  • Experience providing psychotherapy in outpatient settings
  • Experience with geriatric, chronic pain, health psychology, or medically complex patients strongly preferred
All Applicants:
  • Strong clinical documentation skills
  • Collaborative, patient-centered approach
  • Interest in integrative care models
  • Ability to work independently within defined acuity parameters

  • Cognitive Behavioral Therapy (CBT)
  • Acceptance and Commitment Therapy (ACT)
  • Behavioral activation
  • Chronic pain coping skills training
  • Trauma-informed care
  • Adjustment-related counseling
