Care Management Support Coordinator II
Location: Remote (McAllen and Corpus Christi, TX residents ONLY)
Pay Rate: Starting at $18.50/hour (based on experience)
Schedule: Monday-Friday | 9:00 AM - 5:00 PM
Overtime: Available as needed
Dean's Professional Services is actively seeking a Care Management Support Coordinator II to support member services and compliance initiatives within a collaborative care management team. This fully remote, weekday opportunity offers stability, competitive pay, and the chance to make a meaningful impact on healthcare access and service coordination.
Responsibilities- Provide administrative care management support, including outbound member outreach, inbound call handling, and service scheduling
- Serve as a primary point of contact for members, providers, and internal staff to resolve inquiries and issues in accordance with state and regulatory guidelines
- Conduct outreach to support care plan next steps, member education, and coordination of community or health plan resources
- Assist members in accessing benefits and local resources, including referrals addressing Social Determinants of Health (SDOH)
- Support member onboarding activities, including welcome letters, correspondence, and program educational materials
- Accurately document and maintain non-clinical member records in compliance with policies and standards
- Apply working knowledge of assigned health plan activities, procedures, and protocols
- Perform additional duties as assigned while complying with all policies and standards
- High School Diploma or GED (required)
- 1-2 years of medical or healthcare-related experience
- Customer service experience; call center or high-volume call experience strongly preferred
- Comfortable handling frequent phone interactions throughout the workday
- Strong computer skills, including Microsoft Outlook and Excel
- Ability to multitask effectively with strong communication and organizational skills
