• Current, unrestricted Colorado Occupational Therapist license
• Graduation from an accredited Occupational Therapy program
• Valid driver’s license and reliable transportation
• Ability to practice independently in a home-based clinical setting
• Proficiency with, or willingness to learn, Homecare Homebase (HCHB)

• Perform assigned occupational therapy visits, including evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits.
• Develop and implement individualized occupational therapy plans of care based on patient needs, functional goals, and physician orders.
• Provide skilled interventions such as Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and home safety and fall-prevention strategies.
• Educate patients and caregivers on treatment plans, adaptive techniques, home programs, and safety strategies.
• Communicate patient progress, concerns, and changes in condition to the agency's interdisciplinary care team.
• Collaborate in the clinical oversight of Occupational Therapy Assistants (OTAs), when applicable and permitted under Colorado law and agency policy.
