Care Coordinator
Job Title: Care Coordinator
Department: Care Coordination/Clinical Admin
Reports To: Care Coordination Manager
FLSA Status: Exempt
Position Summary
Responsible for essential administrative tasks in relation to Care Coordination. This role involves reviewing patients’ multi-disciplinary services and ensuring compliance with agency policies and regulations.
Key Responsibilities- Effectively manage and organize the Care Coordination spreadsheet.
- Gather, collect, and document coordination of multi-disciplinary services.
- Accurately complete and upload care coordination forms into the patient chart in the EMR.
- Manage incoming and outgoing faxes and emails in relation to care coordination.
- Effectively prioritize required tasks with clinical staff.
- Communicate effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
- Understand regulations and requirements for Medicaid Home Health services.
- Perform other related duties and responsibilities as deemed necessary.
- Educate patient and families on regulations and care coordination.
- Maintain clinical documentation expectations.
- Support and assist patients and families with care coordination.
- High School diploma or GED;
Associates degree preferred. - Demonstrate excellent interpersonal skills, both written and verbal, time management, and Microsoft Office skills.
- Self‑motivated and should need minimal supervision.
- Detail‑oriented and organized.
- Ability to multi‑task competing priorities.
- Minimum of one (1) year experience in a medical office environment.
- Medicaid Waiver and/or Medicaid Home Health knowledge/experience preferred.
- Knowledge of healthcare environment.
- Professionalism and leadership.
- Communication and training.
- Process improvement and compliance mindset.
- Full‑Time Hybrid position; 1-2 Days in office, 3-4 Days Work from Home.
- Flexible Schedule.
- On‑job continuous education provided.
