NJ Palliative Care Solutions is seeking a highly capable Advanced Practice Nurse (APN) to serve as Director of Clinical Operations, supporting the Chief Nursing Officer (CNO) in driving operational performance, productivity, and patient retention across NJPCS’s statewide palliative care programs.
About the RoleThis role is responsible for translating clinical strategy into repeatable, enforceable operating systems that ensure high provider productivity, strong patient retention within NJPCS’s preferred partner network, and consistent execution across inpatient, outpatient, home-based, and post-acute settings. The Director of Clinical Operations owns the day-to-day operational performance of the program, including workflow design, provider utilization, retention tracking, and the systems required to manage care across settings.
This position functions as the CNO’s primary internal execution partner, accountable for identifying operational gaps, implementing workflow fixes, and ensuring that clinical teams operate with discipline and consistency as the organization scales.
Key Responsibilities- Develop and maintain clinical and operational policies and procedures to ensure consistent care delivery, documentation, billing workflows, and regulatory compliance
- Implement and manage clinical workflows, referral pathways, triage processes, and scheduling models across all care settings
- Lead provider productivity, utilization, and patient retention efforts across NJPCS programs
- Monitor and analyze operational metrics, including census, referrals, productivity, discharges, and coverage
- Create and maintain standardized dashboards and reports to provide real-time performance visibility to APNs, site leaders, and the CNO
- Identify performance gaps early and partner with clinical leadership to implement corrective actions
- Support onboarding, orientation, and ongoing operational guidance for advanced practice providers
- Set and enforce productivity standards, documentation requirements, and workflow expectations
- Prepare for and lead specialty certification and accreditation efforts (TJC, DNV, and others)
- Support quality improvement initiatives and clinical data collection
- Proactively identify operational and clinical challenges and implement effective solutions, escalating to the CNO when needed
- Licensed Nurse Practitioner (APN) in New Jersey (Required)
- 3–5+ years of experience in palliative care, hospice, geriatrics, or complex care settings
- ACHPN Board Certified preferred; certification required within one year of hire
- Demonstrated ownership of multi-site clinical operations
- Experience building and enforcing repeatable workflows across care settings
- Proven accountability for provider productivity, utilization, and patient retention outcomes
- Strong organizational, communication, and problem-solving skills
- Experience working across hospital, SNF, and home-based care settings preferred
- Proficiency with EMRs (Epic, Point Click Care , Home Care Home Base preferred)
