Overview

Vantage Point is seeking an experienced and relationship-focused Rehab Field Liaison/Admissions Specialist to support chemical dependency rehabilitation admissions. This remote position includes field-based travel up to a 250‑mile radius to build and maintain referral relationships and support timely admissions. The hybrid role combines field‑based referral development with hands‑on admissions coordination, serving as a key link between referral sources and internal clinical teams.

The ideal candidate will manage referral intake, conduct initial clinical reviews, educate referral partners on program criteria, and coordinate timely admissions while building strong relationships with hospitals, social workers, case managers, and community partners.

Responsibilities
  • Collaborate closely with hospital discharge planners, social workers, case managers, physicians, and management, providing education to generate referral sources.
  • Develop and maintain strong relationships with key referral sources.
  • Identify opportunities for partnering and growth.
  • Manage leads from referral sources and ensure an efficient admissions process for clients.
  • Increase the number of referral sources within a facility, including physicians, social workers, and discharge planners.
  • Coordinate with the clinical assessment team on referrals for admission based on the services provided by the facility.
  • Educate patients, families, community, and other referral sources on programs, services, and amenities provided within the facility.
  • Coordinate the completion of the admission process and ensure appropriate resources are obtained.
  • Travel frequently within the community, scheduling meetings and developing contacts/referral sources.
  • Document referral sources within an appropriate contact‑management system.
  • Develop events and educational forums that demonstrate programs and provide opportunities to identify potential referral sources.
  • Utilize analytic tools, such as Salesforce, to plan and evaluate activity and optimize results.
  • Represent the facility in professional forums and communicate the mission in all outreach interactions.
  • Create outreach initiatives that regularly communicate the facility mission with referral sources through phone, email, e‑blast, written notes, events, networking, and other professional activities.
  • Provide the facility or receiving team with appropriate information to facilitate admission when necessary.
Qualifications
  • High school diploma or equivalent required.
  • Bachelor's degree in Marketing or a related health or social services field preferred.
  • Alternative educational credentials:
    Bachelor’s in Behavioral Science or Social Services; RN with an Associate’s in Nursing;
    Licensed Practical or Vocational Nurse with LPN or LVN training;
    Master’s in a therapeutic field preferred for applicable states.
  • One or more years of experience working with individuals in a clinical or observational capacity within the specialty area of the facility preferred.
  • Two or more years of experience in sales and/or marketing preferred.
  • Must have and maintain a valid driver’s license.
