The full‑time Options Counselor offers flexibility to work from your home office and make in‑person visits to participants who reside within your assigned territory.

WhatWill You Do?
In this community‑based program, the Options Counselor provides person‑centered long‑term services and supports options counseling at the setting of the participant’s choice, facilitates applying for benefits and services, and provides resources to address the participant’s needs and preferences to live independently.
Responsibilities- Provide participant‑directed and interactive interviews with the participant to develop a person‑centered action plan for long‑term services and supports within one month of referral.
- Provide education, information, and resources based on the participant’s preferences, needs, and goals.
- Facilitate access to private sector services and supports and public programs, as well as assistance with completion of benefit enrollment applications, as applicable.
- Provide options counseling at the setting of the participant’s choice including in person at the participant's home, hospital, nursing facility, assisted or independent living, community setting, via phone or via video conference.

- Comfortable with navigating various technologies and computer applications
- Team player mentality
- BSW or MSW licensure (limited or full) in the State of Michigan (Master’s preferred); or
- Have a bachelor’s degree in a health or human services field and at least 3 years of experience in the provision of health or social services; or
- A Community Health Worker certification with at least 3 years of experience in the provision of health or social services.
