Aspiranet is seeking a compassionate and driven Lead Care Manager (LCM) to join our Enhanced Care Management (ECM) Program. In this role, you will provide direct services while coordinating comprehensive, client-centered care as part of a multidisciplinary Care Team.

📝 Position Summary

Under the direction of the Core Program Supervisor/Manager, the Lead Care Manager provides direct services and coordinates comprehensive care management for assigned clients in the Enhanced Care Management Program (ECM).

The LCM functions as part of an interdisciplinary Care Team, overseeing specific cases and ensuring clients receive timely, cost-effective access to appropriate health and social services. This position requires strong advocacy, collaboration, and community engagement skills to maximize client outcomes and benefits.

🔍 Key Responsibilities
  • Conduct comprehensive assessments of client needs, including physical health, mental health, substance use, oral health, trauma history, housing stability, social supports, vocational/employment goals, and overall wellness.
  • Develop, implement, and monitor Individual Care Plans and Health Action Plans, ensuring goals are client-centered and regularly updated based on progress.
  • Provide care management services, including service coordination, referrals, resource linkage, and follow-through on care plan activities.
  • Coordinate and maintain communication among clients, caregivers, healthcare providers, hospital staff, and community-based organizations.
  • Advocate on behalf of clients with primary care providers and other service professionals to ensure timely and appropriate access to services.
  • Support transitional care planning, including collaboration with hospital staff during admissions and discharges.
  • Provide services in community settings and client homes as appropriate to support engagement and accessibility.
  • Accompany clients to critical appointments when necessary and assist with arranging or providing transportation as indicated.
  • Deliver health education, coaching, and skill-building activities that promote independence, resilience, and overall wellbeing.
  • Utilize evidence-based practices, including Motivational Interviewing, Harm Reduction strategies, and Trauma-Informed Care principles.
  • Conduct outreach and engagement activities to facilitate enrollment in the Enhanced Care Management program, including phone outreach, mailed communication, field visits, and community presentations.
  • Document services accurately and in a timely manner in accordance with program standards, funding requirements, and agency policies.

  • High School Diploma or GED required.
  • Minimum of two (2) years of relevant work experience.
  • Resident of the community with strong knowledge of local resources.
  • Ability to complete required training and ongoing educational requirements.
  • Ability to work flexible hours, including occasional evenings and weekends.
  • Valid driver’s license, reliable transportation, and proof of insurance (if required for field-based services).
