Care Coordinator – Licensed Practical Nurse (LPN – Senior Care)

Location: Woodstock, Georgia

Job Type: Full-time

Hours: Monday–Friday 9 am–4 pm in the field, patient‑facing. Remaining time for admin work from home.

Compensation: Competitive base pay plus bonus pay.

All equipment will be provided. No nights or weekends.

Position Summary

We are seeking a dedicated Care Coordinator – Licensed Practical Nurse (LPN) to support a healthcare provider visiting Senior Care Communities. The Care Coordinator will serve as the scribe and the hub of communication, ensuring smooth operations between the provider, the facility staff, and the patient’s family members, emergency contacts, or power of attorney when necessary.

Key Responsibilities Scribe & Documentation
  • Act as the primary scribe for the provider during patient visits at the facility, documenting in Athena EMR in real‑time.
  • Record patient encounters: history, physical exam findings, assessment, plan, medications, and follow‑up instructions.
  • Update patient records promptly to ensure continuity of care.
  • Monitor and track patient progress, documenting changes to medications, care plans, and health conditions.
  • Ensure all documentation complies with privacy and confidentiality guidelines (HIPAA).
Communication Hub
  • Serve as the primary communication point between the healthcare provider and the facility staff.
  • Ensure timely communication of patient needs, updates, and concerns.
  • Collaborate with staff to coordinate follow‑up appointments, medication changes, and referrals.
  • Provide clear instructions to staff and patients, ensuring understanding of orders and treatment plans.
Communication with Family Members, Emergency Contacts, & Power of Attorney
  • Communicate important updates regarding care to family, emergency contacts, or POA as designated.
  • Ensure family members remain informed of significant changes while maintaining confidentiality and HIPAA compliance.
  • Act as a liaison to facilitate understanding and coordination between provider and family or POA.
  • Assist in coordinating care‑related requests or concerns from family members.
Provider Support & Schedule Management
  • Assist in managing the provider’s daily schedule and coordinating patient visits.
  • Maintain and update the provider’s calendar for an efficient workflow.
  • Coordinate follow‑up visits, tests, and referrals with the provider.
  • Prepare necessary documentation and information for upcoming patient visits.
Patient Care Coordination
  • Facilitate patient access to care by ensuring all necessary forms, consent documents, and patient information are prepared ahead of each visit.
  • Schedule required follow‑up appointments or referrals as per the provider’s recommendations.
  • Review medication lists and reconcile medications in Athena.
Clinical Support
  • Assist the provider with patient exams and provide necessary tools and equipment.
  • Obtain and record patient vitals such as blood pressure, weight, temperature, and oxygen levels when applicable.
  • Provide patient education, offering information on new treatments, medications, and care instructions.
Other Duties As Assigned
  • Perform other duties as assigned by the healthcare provider or supervisor.
