Authora Care Collective is currently seeking an RN Care Continuum Navigation Specialist for our Greensboro campus. This is a full-time hybrid position (Monday - Friday 8:30am - 5:00pm).
* Must reside in the Burlington/Greensboro area
* CPR Certification required.

Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area.

We are seeking a dedicated and compassionate RN Care Continuum Navigation Specialist to join our team. This role will focus on guiding patients through various internal care programs including counseling, dementia care, home health, home-based primary care, hospice, Medicaid waiver services, and palliative care as well as any future programs. Additionally, this position will play a key part in managing eligible patients in collaboration with referral partners, overseeing the assessment and implementation of care coordination activities aligned with value-based care best practices.

This role will be pivotal in connecting all internal and external communications with stakeholders related to patient care.

The RN Continuum Care Navigation Specialist will be responsible for knowing all parameters and protocols for patient eligibility for all programs offered within Authora Care Collective and with aligned partners. A key function is identifying timeliness of admission into programs benefiting both patients and payer sources, ultimately benefiting Authora Care Collective as a serious illness organization.

Education/

Qualifications:

* Associate or bachelor's degree nursing

* Current registered nursing license in North Carolina

* Two years' registered nursing experience required

* Two years of experience in healthcare case management preferred
· Case management accreditation or certification through ACM-RN or CCM strongly preferred or obtained within 1 year of hire

* Other:
Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation

Key Responsibilities:

* Comprehensive Case Management Assessments:
Assess patient needs, create and update individualized care plans identifying care needs that lead to high risks for emergency room visits and hospitalizations. Identification and risk stratification for level of care changes or program progression within a care continuum is paramount

* Care Coordination:
Collaborate with internal and external teams/partners and community resources to support patients in navigating home health, hospice, home-based primary care, counseling, Medicaid waiver services and dementia care services promoting navigation through Authora Care Collective's continuum of services at the right time for appropriate patient and organizational needs and priorities. Knowledge of any other care coordination service(s) outside of Authora Care Collective's umbrella which promotes quality, overall cost control, and value based care principles is required.

* Patient Advocacy:
Act as a liaison between patients, families, and internal healthcare team, ensuring patients have access to necessary resources and services while advocating for their rights and preferences while simultaneously balancing the needs and resources of Authora Care Collective.

* Internal Resource Management:
Maintain updated knowledge of internal resources and services appropriate for each service line leading internal stakeholders/peers through guidance on patient eligibility for programs related to patient need for timely referrals and movements.

* Internal Referral Specialist:
Provide patients and families with information and referrals to appropriate internal programs.

* Maintain contact throughout the referral process to ensure appropriate enrollment in all eligible services.

* Patient Relationships:
Establishes and maintains relationships with patients/caregivers to adequately assess ongoing needs as needed along the internal care continuum.

* Cultural Competence:
Recognizes influence of culture, ethics, and beliefs on decision making.

* Quality Improvement:
Participate in quality improvement initiatives to enhance service delivery, patient satisfaction, and outcomes.
